TAP Forms

Please download and complete all forms. All applications should be submitted in a single PDF document that includes the Application form, completed W9, quad chart, white paper and budget in that order. The file should be named: BusinessName_Technologycategory.pdf and should be submitted to USTAR@utah.gov. The “technology category” refers to the category listed in the official announcement (Automation and Robotics, Advanced Materials, Energy and Clean Technology and Life Sciences). Electronic submissions via email are preferred. A paper copy with an electronic copy on a memory stick may be submitted to the front desk of 60 E South Temple, 3rd floor, Salt Lake City UT 84103.

All forms must be completed and returned to be considered. Refer to the Official Rules and to the announcement for more details.

Questions regarding TAP must be submitted in writing via email. Inquiries made via phone or other method will not be accepted.
